How to get the COVID-19 vaccine in Indiana?

If you are between ages 12-15, you need an adult to accompany you to the vaccine appointment. If the adult present is not the parent or guardian, consent must be submitted prior. If you are 16 or 17 years old, you can take the parental consent with you. You don't need to be accompanied with an adult to the clinic.
