How to get the COVID-19 vaccine in Massachusetts?
Parental consent is required but parents do not need to be at the clinic while the teen is receiving their vaccine. If the parent or guardian is not accompanying the minor, they should download and complete a pre-vaccination screening form, available at mass.gov/CDCScreeningForm.
